Learning Objectives: At the conclusion of the program, participants will be able to describe best practices for patient care in post-acute and long-term care, summarize clinical and practice updates as related to the post-acute and long-term care setting, and describe methods to support teamwork through inter-professional teams in the post-acute care long-term care environment that are designed to result in quality care and increased satisfaction of the residents, their family and the staff.

CMD: This live activity has been pre-approved by the American Board of Post-Acute and Long-Term Care Medicine (ABPLM) for a total of 1 management hour and 3 clinical hours toward certification as a Certified Medical Director (CMD) in post-acute and long-term care medicine. The CMD program is administered by the ABPLM.  Each physician should claim only those hours of credit actually spent on the activity. 

Nursing Contact Hours: This continuing education activity was approved by the Ohio Nurses Association for 4 contact hours including 4 contact hours of advanced pharmacotherapy. The Ohio Nurses Association is accredited as an approver of continuing nursing education by the American Nurses Credentialing Center's Commission on Accreditation.
